Data Superstars: Nursing Informatics in Post Acute Care

This session will discuss the variety of roles of the informatics nurse in the post acute care setting. The session will cover the opportunities to improve care through the use of data including government data sets and quality measures. It will also discuss the challenges of transitions of care through the PAC setting.
