This session showcases a simple, innovative solution to engage patients and families in the patient plan of care through the creation of the Daily CARE (Communicate, Access, Resource, and Educate) Plan. Discussion of the plan, which ensures the patient/family is informed of the plan of care for the day, represents a true partnership with patients and their families, and opens up new and creative ways every nurse can explore using technology to best partner with patients.
