134 - Automating Data Gathering to Assist Home Health Clinicians with Planning Care to Reduce Re-Hospitalization

By using data collected at start of care, a large home care agency was able to automatically compile a hospitalization risk score and impart that information to visiting clinicians. This process, which included best practices that were implemented to prevent patient readmission to the hospital, is discussed, and lessons learned are shared.
