The session focuses on understanding the consequences of a fragmented healthcare system, the impact of social determinates of health (SDOH) and health equity on health outcomes, the key elements necessary for effective care coordination, the importance of team-based care, and ways in which technology enables the application of evidence-based practice and improves care coordination.
