Poor disease management of chronic conditions is a problem that can have negative effects on a person’s health. Many factors can contribute to this problem including financial concerns, confusion, and poor communication between care team and patient. This adds to medical costs and poor patient outcomes. There has been some research to explore the use of web-based programs and its use in modern medicine. The use of patient portals is a relatively new technology that could provide unique opportunities to improve patient care and healthcare outcomes in those with chronic disease. This is a literature review of articles and studies that defines the use of a patient portal and its integration into the management of chronic diseases.
